Inspector Notes

A monitoring inspection was initiated on 12/09/21 and concluded on 05/05/22 in response to a self-reported incident regarding the supervision of children and forbidden action. The Department was notified of the incident on 12/03/21. On 12/09/21, the inspector was on site with a local agency from 2:20pm to 4:45pm. Interviews were completed, observations were made at the center, and documentation was reviewed. The inspector completed additional interviews and follow-up activities at the facility on 02/02/22 from approximately 2:40pm to 3:55pm. The preponderance of the evidence gathered during the investigation did not support violations of the standards. There are no violations resulting from this inspection.
