Inspector Notes

An unannounced complaint inspection was initiated on Thursday, June 1, 2023, and concluded on Tuesday, July 18, 2023. The complaint was regarding allegations of non-compliance with the physical plant and building maintenance. On June 1, 2023, the inspector as on-site from approximately 11:00am until 11:45am. Interviews with staff were completed, observations were made at the center, and documentation was reviewed. Additional documentation was requested and reviewed remotely. On Thursday, June 15, 2023, the inspector was on-site from approximately 9:00am until 2:05pm conducting follow-up activities and interviews.

The preponderance of evidence gathered during the investigation does not support the allegation; therefore, the complaint was determined to be not valid. There were no violations resulting from this inspection.
