Inspector Notes

An unannounced complaint inspection was initiated on Monday, March 4, 2024, and concluded on Wednesday, May 1, 2024. The complaint was regarding the alleged non-compliance in the areas of staffing and supervision; injury reporting; programming; and administration. On March 6, 2024, two inspectors were on-site from 2:15pmuntil approximately 3:45pm. Interviews with staff were completed, observations were made at the center, and documentation was reviewed. Collateral interviews were conducted on March 4, 2024.

The preponderance of evidence gathered during the investigation does not support the allegations; therefore, the complaint was determined to be not valid. There were no violations resulting from this inspection.
