Inspector Notes

An unannounced monitoring inspection was initiated on 12/03/2024 and completed on 12/03/2024 in response to a self-report received by the licensing office on 11/27/2024 relating to injury reporting. The inspector reviewed one child?s record and one staff record on-site. This inspection included document review, tour of the facility, interviews, and observations.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 01/09/2024. A POC submitted after this date will not appear on the public website.

The center shall notify the parent by the end of the day of any known minor injuries.

On 11/26/24 at approximately 4:15pm, Child #1 (3-year-old) sustained an injury on the playground. Staff #1 witnessed the injury, but did not notify the child's parent when the child was picked up.
Plan of Correction: Per the Center: "Retrained all staff members at staff meeting on 12/30/2024."
