During the inspection, the inspector reviewed the areas listed above. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced, on-site renewal inspection was conducted 2/4/2025 from 10:45am - 12:55pm. The inspector reviewed compliance in the areas listed above. There were 9 children and 2 caregivers present during the time of the inspection; total 19 points. The inspector reviewed 3 child records, 2 provider/caregiver records, and 3 household records onsite. The inspection included document review, interviews, observations, and a tour of the facility.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the deficient practice will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on February 13, 2025. A POC submitted after this date will not appear on the public website.

Every adult living in the family day home shall: authorize the family day home to obtain a copy of the results of a search of the central registry for any founded complaint of child abuse or neglect against him prior to issuance of a license and every five years thereafter.

Household member #1 had central registry results that were expired for 3 months before a new search request was made.

The physical examination prior to attendance shall have been conducted within: 3 months prior to attendance for children age 7 months through 18 months.

The physical for Child #2, age 14 months at the time of enrollment, was conducted 5 months prior to attendance.

Potentially poisonous substances, materials and supplies such as, but not limited to, cleaning agents, disinfectants, and deodorizers shall be stored away from food in areas inaccessible to children.

A container of Clorox wipes and Lysol wipes were found underneath the bathroom sink in an unlocked cabinet, making them accessible to children. Clorox wipes were found on the kitchen counter, near the stove top and food prep area.
Plan of Correction: The cleaning supplies on the kitchen counter were removed during the inspection.

A landline telephone, excluding a cordless or cell phone, shall be available, operable, and accessible during the family day home's hours of operation.

The family day home did not have a landline telephone.

Occupied cribs, cots, rest mats, and beds shall be at least 12 inches from each other.

During the designated rest period, 4 children were observed sleeping in a crib or on a rest mat that was less than 12 inches from each other.

Before a child may attend the family day home, the provider shall obtain documentation that the child has been adequately immunized according to the requirements of the Code of Virginia and applicable State Board of Health Regulations.

The immunization record for Child #1 was dated 14 days after the first date of attendance.
