Mattresses shall be covered with a waterproof material that can be cleaned and sanitized.
On the date of the inspection, 4 out of 6 crib mattresses had a ripped or torn covering which exposed the inside padding. An infant was observed in 1 of the cribs with the ripped covering.

Plan of Correction: Only 2 cribs need to be used at this time. The broken mattresses will not be used and replaced before those cribs are put back in use.
