Inspector Notes

The licensing inspector conducted an unannounced Subsidy Health and Safety inspection on September 21, 2023 from approximately 9:45am to 12:45pm. There were 16 children (two to five years of age) with four staff supervising. A total of five children and five staff records were reviewed. A code compliance inspection was conducted on the same date as the SHSI supplemental inspection.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Standard 8VAC20-790-550-2-f
Based on review of five staff records and interview, the vendor did not ensure one staff member obtained documentation of subsequent background checks at least every five years.
Evidence:
1. The record for Staff #2 (first day of employment 10/27/2017) contained a fingerprint based national criminal record check dated 08/22/2028.
2. Staff #2 confirmed as of 09/21/2023 an appointment to obtain an updated background check had not been completed.
Plan of Correction: An appointment to update the background check was scheduled while the licensing inspector was on site.
Standard 8VAC20-790-560-A
Based on review of five staff records, the vendor did not ensure one staff member obtained a negative tuberculosis (TB) screening within the required timeframe.
Evidence:
The record for Staff #4 did not contain documentation of a TB screening.
Plan of Correction: I will have the staff member schedule an appointment to obtain a TB screening.

Based on observation and interview, the vendor did not ensure all areas and equipment of the center, inside and outside, were maintained in a clean, safe, and operable condition.

Evidence:
1. Several areas of exposed landscaping fabric were observed on the children?s playground. The exposed fabric created a tripping hazard.
2. A member of management was shown the hazards during the inspection. The member of management confirmed these observations.
Plan of Correction: I will have the maintenance team remove the exposed fabric from the playground.
Standard 8VAC20-790-640-A
Based on observation, the vendor did not ensure all hazardous substances such as cleaning materials, insecticides, and pesticides were kept in a locked place using a safe locking method that prevented access by children.
Evidence:
1. Containers of cleaning products were in an unlocked cabinet in the Threes classroom.
2. Cleaning chemical were located on the floor in the director?s office.
3. Children were present in the Threes classroom and the director's office during the inspection.
Plan of Correction: The cleaning chemicals will be moved to an area that can be locked.
