Inspector Notes

An unannounced code compliance inspection was conducted on 7/19/2023 from 9:30am-11:10am. During the inspection there were 24 school age children in care with two staff and two volunteers. Children were observed participating in various activities. Records were reviewed for three staff and 5 children. The vehicle used for transporting children on field trips was observed as well as field trip procedures. Information gathered during the inspection determined noncompliance with applicable standards or law and violations were documented on the violation notice issued to the program, and were discussed during the exit interview

Based on observation and interview the center did not ensure that there was a posting that the center was exempt from licensure in a visible location on the premises.
Evidence:
The center did not post the fact that it is exempt from licensure in a visible location on the premises. Staff #2 stated she did not see this posted on the premises.
Plan of Correction: The center will post the statement that the program is exempt from licensure.
Standard 22.1-289.031-A-4
Based on a records review the center did not ensure that each person in a childcare supervisory role has been certified by a practicing physician or physician assistant to be free from any disability which would prevent them from caring for children under their supervision.
Evidence:
The record for Staff #1 did not contain a staff health report certifying that he staff member is free from any disability that would prevent her from caring for children.
Plan of Correction: The staff member will have a physician complete the staff health form.
