During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.
Inspector Notes
A subsidy health and safety inspection (SHSI) was conducted in conjunction with the licensing inspection. Subsidy standards that are not covered in licensing standards were reviewed during this SHSI. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Violations
6The records for Child #1, Child #2, Child #3, and Child #4 did not include all the information required to be in a child's record.
The TB screening for Staff #2 was expired on the day of the inspection.
The records for Staff #1 and Staff #2 did not have current CPR certification, which is required for all staff.

Staff who work directly with children shall annually attend at least 16
hours of training and staff development activities, to include the department's health
and safety update course.
The records for Staff #1, Staff #2, and Staff #3 did not contain documentation that 16 hours of annual training was completed from September 2024-May 2025, which is the center's annual training schedule. In addition, the department's health and safety course was not completed in 2024 for Staff #1, Staff #2, and Staff #3.
Hazardous substances such as cleaning materials, insecticides, and pesticides shall be kept in a locked place using a safe locking method that prevents access by children.
There was a container of disinfectant wipes on a desk and paint spray on top of a refrigerator. Both substances were left unlocked and had a manufacturer's label that stated "keep out of reach of children."
