Inspector Notes
An unannounced monitoring inspection was conducted on 1/12/23 at 10:46am. There were 43 (6 months to 5 years) directly supervised by 8 staff. The physical plant, 5 staff records, 5 children?s records, evacuation drills, injury reports, emergency supplies, and policies were inspected. Children were observed participating in group play, listening to stories, and playing outside. Hand-washing procedures were also observed. There was a sufficient supply of books, toys, and materials for the children. There was an adequate number of staff with current certification in MAT, CPR and First Aid, as well as DHO training. Staff-to-child ratios were in compliance. The center was clean and organized. Areas of non-compliance are identified in this report. Violations
4Evidence:
1) observed an outdoor air vent that was broken and out of place, accessible to children.
2) observed 2 separate cracks in the playground fencing
3) observed 2 propane tanks in an unlocked cabinet, in the school-age classroom
Evidence: observed a cabinet with cleaning agents inside, but the key was in the lock and accessible to children.
Evidence: reviewed 5 children's records and found that Child 2 did not have documentation of the parents annual review of emergency contact information.
Evidence: reviewed 5 staff records and found that Staff 3 and 4 did not have documentation of staff references completed prior to employment. Staff 3 was hired on 12/22 and references were conducted on 12/29. Staff 4 was hired on 12/19 and both references were conducted on 12/22.
