Inspector Notes

This inspection was conducted by staff using an alternative remote protocol, including phone interview, records review, and a virtual observations. An on-site inspection was not conducted at this time.

An unannounced monitoring inspection was conducted at 10:25am. The director was contacted by phone to initiate the inspection, and given a list of items required to complete the inspection. There were 70 children and 17 direct-care staff present at the time of inspection. Ratios and supervision were in compliance. The facility was well supplied with toys and equipment for the children, and appeared to be clean and organized. Information gathered during the inspection determined there were areas of non-compliance with applicable standards or laws reviewed. Standard 22VAC40-191-60-C-2
Based on records reviewed, it was determined that 1 of 2 staff records reviewed did not include a required background check. Evidence: Staff #6 - There was error made when submitting the central registry check (CPS) and therefore the results of said check are not valid.
Plan of Correction: Staff #6 will another CPS check, and the results will be sent to the inspector for review.

Based on observations made, it was determined that the most recently issued violation notice (inspection report) was not posted in the facility for public viewing.
Plan of Correction: The report will be posted in the lobby of the school, in a location determined suitable by the program director.
