The center is required to keep a current list of all known allergies, sensitivities, and dietary restrictions in each room or area where children are present. The list must be dated and kept confidential.

The allergy lists in 7 out of 8 classrooms were posted on their classroom board and not kept confidential.
Plan of Correction: We have updated the allergy List to include only the first initial of each child and displayed the corrected list in the classrooms to maintain confidentiality.

Staff shall request at least annually parent confirmation that the required information in the child's record is up to date. Up-to-date information is required to be requested from the parent for each child's record at least annually.

The information in the record of Child #5 had not been updated in over 13 months which is overdue by one month.
Plan of Correction: We provided parents with a hard copy of the forms to complete for the 2025?26 school year.

There shall be at least 12 inches of space between occupied cots, beds, and rest mats.

During nap time, 5 cots in Classroom #1 were spaced 6 inches apart and not the required minimum of 12 inches.
Plan of Correction: We have reviewed the regulations with all teachers. They are ensuring the entire classroom space is used, and that cots are placed with 12 inches of spacing between them.

Centers must keep a separate record for each child enrolled that contains all of the required information.

The record for Child #5 (enrolled 1 year) was missing documentation of viewing proof of the child's identity and age.
Plan of Correction: We reviewed the forms and asked the parent to complete them
again, as the format was mixed up. We completed the section requiring proof of the child?s identity and age. We are reviewing all files to ensure nothing is missing.
