Inspector Notes
This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review, interviews, and a virtual tour of the program.
A renewal inspection was initiated on 9/14/2021 and concluded on 9/20/2021. The Director was contacted by telephone and a virtual inspection was conducted. There were 21 children present, ranging in ages from 3 to 5 years, with 10 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 2 child records, 2 staff records, and 5 board member records were reviewed.

1Evidence: Board Member #2 (start date: 3/18/2021) did not have a copy of the results of a criminal history record information check, a sex offender registry check, and a search of the child abuse and neglect registry or equivalent registry from state #1 in which the individual has resided in the preceding five years.
