Inspector Notes

Today an unannounced Monitoring Inspection took place between the hours of 11:05am and 12:10pm. There was a total of 27 children with 8 staff (and multiple co-op parents) within the supervision guidelines. The children were observed playing outside on the playground, riding trikes and other organized activities. A complete inspection of the physical plant, children and staff/co-op parent records, evacuation drill log, and emergency materials were observed during this inspection. Violations were found during today?s inspection and can be viewed on the Violation Notice. Thank you for your cooperation during the inspection. Based on a review of staff records, 1 out of 3 staff did not have results of a follow-up tuberculosis screening at least every two years from the date of the first initial screening or testing.

Evidence:
1. Staff #1 (doh: 9/19/2018) did not have documentation of a tuberculosis screening on file during the time of the inspection. The last tuberculosis reviewed was dated 8/18/2020.
Plan of Correction: The staff member went to get a new tuberculosis screening on the same day of the inspection.
