Inspector Notes

This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review and interviews.

A monitoring inspection was initiated on 12/02/2021 and concluded on 12/06/2021. The provider was contacted by telephone and a virtual inspection was conducted. There were 9 children present, ranging in ages from 6 months to 2 years, with 3 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 2 child records and 1 staff record were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Standard 22VAC40-111-870-A
Based on documentation review, the menu for the one week period did not contain all required information.
Evidence:
1) The morning snacks listed in the menu were missing one nutritional component. The provider stated that the second component would be milk that they offer everyday but it was not listed in the menu.
Plan of Correction: I forgot to include the milk in the menu. We have decided that, moving forward, we will not offer the morning snack since we offer breakfast and will move up lunch to an earlier time.

Based on record review and interview with the provider, it was determined that the provider did not ensure to obtain a fingerprint-based criminal history check determination letter prior to first day of employment for one caregiver.
Evidence:
1) The record for Staff #3 (start date 09/27/2021) contains a fingerprint-based criminal history check determination letter dated 10/26/2021.
Plan of Correction: I will ensure to have the fingerprints back before employment for future hires.
