Inspector Notes
A monitoring inspection was conducted on 05/17/2023 between the hours of approximately 11:40 am and 12:45 am. There were 12 children present with 3 caregivers supervising.
The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 3 child records, 3 staff records and 1 household member record were reviewed.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Violations
7Evidence:
1) On the date of inspection, the most recent license for the program was not posted. The license posted had expired in January 2023.
Evidence:
1) One outlet in the bathroom of the care area did not have a protective cover.
Evidence:
1) On the date of inspection, the record of attendance was not current. The weekly attendance record showed attendance for 05/15 and 05/16. The attendance for 05/16 and 5/17 was not recorded.
Evidence:
1) On the date of inspection, an infant was positioned on a bouncy chair and the safety straps were not fastened.
Evidence:
1) On the date of inspection, while the children were playing outside, the inspector observed two children in infant walkers. While inside the fdh, the inspector observed one child in an infant walker.
We removed the wheels of the walker which is outside. The walker inside doesn't have wheels, we are just using it as a standing toy. (LI has requested to not use any of the walkers until we get clarification from management.)
Evidence:
1) The record for Child #2 did not contain the address of one custodial parent's place of employment; and the address of a second designated person to contact in case of an emergency if the parent cannot be reached.
Evidence:
1) On the date of inspection, the menu available for review was not dated for the current one-week period. The date corresponded to April 2023.
