Inspector Notes

A subsidy health and safety (SHSI) inspection was conducted on 05/17/2023. There were 12 children present with 3 caregivers supervising. A licensing inspection was also conducted on this date. Both inspections combined cover subsidy health and safety regulations. Only subsidy requirements that are not covered in licensing standards were reviewed during this supplemental inspection.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Based on documentation review, the provider did not maintain an accurate record of daily attendance that documents the arrival and departure of each child in care as it occurs.
Evidence:
1) The record of attendance for the week did not have documented the departure of children for 05/15 and 05/16.
Plan of Correction: Corrected during inspection. The provider updated the attendance record.
Standard 8VAC20-790-250-B
Based on record review, staff did not complete the Virginia Department of Education-sponsored orientation course within 90 calendar days of employment.
Evidence:
1) On the date of inspection, the records for Staff #2 did not contain documentation of having completed the Virginia Department of Education-sponsored orientation course.
Plan of Correction: The provider shall ensure that Staff #2 completes the training within the next 15 calendar days.
