Inspector Notes

This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.
A monitoring inspection was initiated and concluded on 5/27/21. The provider was contacted by telephone to initiate the inspection. There were 10 children (27 points) and 2 caregivers present. The inspector emailed the provider a list of items required to complete the inspection. The Inspector reviewed 2 children?s records, 2 caregiver's records and 1 household member's record submitted by the facility to ensure documentation was complete. Additional daycare forms were reviewed as well. Children were engaged airplane theme-based activities. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the facility. The Licensing Inspector has reviewed with the provider COVID-19 Essential Guidance for Child Care programs. Based on record review, he provider did not obtain for each adult household member a current Report of Tuberculosis (TB) Screening form, every two years from the date of the first screening.
Evidence: Household Member (HHM) # 1's most recent TB screening was dated 8/13/18, and expired on 8/13/20.
Plan of Correction: A TB screening has been scheduled.
