Based on records review, the provider did not obtain for each caregiver a current Report of Tuberculosis Screening form, every two years from the date of the previous screening.
Evidence: Caregiver # 1's most recent Tuberculosis (TB) Screening was dated 10/7/19. An updated TB screening was not obtained.
Plan of Correction: I took my TB test on Oct 13 and right now I have my results.
Standard 22VAC40-111-240-A
Based on observation, furnishings inside the family day home were not maintained in safe condition.
Evidence: Tears and holes were found in some of the soft climbing pillows used for the children to play with, exposing the internal padding and filling.
Plan of Correction: The pillows that are unstitched were removed from teh daycare and replace it with a new one as soon as I can get another gym.
