Inspector Notes
The inspection was initiated on February 21, 2023 and concluded February 21, 2023. There were 3 children present with 2 staff supervising. The inspector reviewed compliance in the areas of administration, personnel, physical health of caregivers, caregiver training, physical equipment/ environment, care of children, preventing spread of disease, emergencies and nutrition. A total of 3 child records, 1 provider and 1 assistant record was reviewed.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program
Violations
4Evidence: The licensee confirmed that the last inspection was conducted on January 26, 2022.
Evidence: A composite piece / play structure was observed without a six foot perimeter from other play structures or obstacles.
1. There was approximately 16 inches between one side of the structure and another caterpillar type play structure.
2. There was approximately 2? 6?-5? 6? between the sides of the structure and the perimeter plastic boarder on all sides.
Evidence: The licensee confirmed they did not conduct emergency evacuation drills during the months of May, June, July, and August 2022.
Evidence: Three children were observed eating lunch and they were served crackers, cheese , ham, cookies and water. The licensee confirmed they only offered 3 of the five components required, by USDA, during lunch time.
