Inspector Notes
The inspection was initiated on May 2, 2023 and concluded May 9, 2023. There were 5 children present, ranging in ages from 7 months to age 3, with 2 staff supervising. The inspector reviewed compliance in the areas of administration, personnel, physical health of caregivers, caregiver training, physical equipment/ environment, care of children, preventing spread of disease, emergencies and nutrition. A total of 5 child records,1 household member, 1 provider and 1 assistant record were reviewed.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program
Violations
4Evidence: The record for Provider #2 did not contain a CRS and thy began working on 2-28-23.
Evidence: The provider confirmed they did not have information available for review regarding orientation for Provider #2 who began working on 2-28-23.
Evidence: Provider #1 confirmed the play pen was used last week for a toddler and there were 2 holes observed in the mesh sides.
Evidence: The records for Child #1 (date of attendance 10-1-23), Child #2 (date of attendance 9-26-22) and Child #4 (date of attendance 9-19-22) did not include written acknowledgements from parents.
