Inspector Notes

A monitoring inspection was initiated and concluded on 9/18/2023.? There were 6 children present with the provider and 1 staff supervising. The point total was 13. The children were observed playing with toys and eating lunch. A total of 5 child records and 2 staff records were reviewed. The inspection started at 10:50 am and concluded at 12:20 pm.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Based on record review and staff interview the licensee did not ensure to obtain that two or more references as to character and reputation as well as competency were checked before employment.
Evidence: The provider confirmed that reference checks had not been completed for Staff #2.
Plan of Correction: Reference checks were obtained and put in employees file on 9/18/2023
Standard 8VAC20-800-70-A
Based on record review and staff interview the licensee did not ensure, before the child?s first day of attendance, parents shall be provided in writing the necessary information.
Evidence: The record for child #1 (First date of attendance 9/12/2023) did not include written acknowledgments from parents.
Plan of Correction: Parent filled out and returned the paperwork on 9/19/2023.
