During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced, on-site monitoring inspection was initiated and completed on 8/19/2025. The on-site inspection began at 9:30 am and ended at 11:00 am. The inspector reviewed compliance in the areas listed above. There were 2 children present and 1 caregiver; the point total was 6. The inspector reviewed 2 children?s records and 3 caregiver/household member(s) records on-site. This inspection included document review, interviews, and observations.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 8/28/2025. A POC submitted after this date will not appear on the public website.

Standard 22.1-289.035-B-2
Providers must obtain a completed national criminal background check prior to the employee's first day of employment. Caregiver #2 did not have a completed national criminal background check.
Plan of Correction: Caregiver is making appointment with field print. They had done fingerprints with the sheriffs office.
Standard 8VAC20-800-120-B
Assistants' and substitute providers' records shall contain all required information.

Caregiver #2, was missing date of employment, and documentation of two or more references as to character and reputation as well as competency were checked before employment.

Documentation of the orientation shall be signed and dated by the provider and substitute provider or by the provider and assistant.

Child #2, enrolled for five months, was missing proof of the child's identity, the home address for one parent, and the name, address, and telephone number of each parent's place of employment.

The diapering surface shall be cleaned and sanitized after each use. The diapering surface was sanitized but not cleaned in such a way as to remove dirt and debris by scrubbing and washing with soap and water.
