(Repeat Violation) Assistants' and substitute providers' records shall contain all required information. Caregiver #2, was missing documentation of two or more references as to character and reputation as well as competency were checked before employment.
Plan of Correction: All required information and documentation have been received and/or updated and placed in the volunteer's folder. Going forward, a checklist will be created for reference to ensure all required documentation for new caregivers Is received at the appropriate time.
Standard 8VAC20-800-60-A
The provider shall maintain an up-to-date record at the family day home for each enrolled child. There was no record for child #1.
Plan of Correction: All records will be updated and maintained for each enrolled child.

(Repeat Violation) Before the child's first day of attendance, the provider shall obtain the parent's written acknowledgement of the receipt of the information regarding family day home operations and administration. Child #2, enrolled for eight months, was missing the information for parents sheet.
Plan of Correction: All required acknowledgements have been signed and are up to date. Going forward, a checklist will be created for reference to ensure all required documentation for new enrollees is received at the appropriate time.

A working portable battery-operated weather band radio, and extra batteries shall be kept in a designated area and be available to caregivers at all times. The portable radio was not operable and there were no extra batteries available.
Plan of Correction: A portable battery-operated weather band radio and extra batteries have been purchased. A designated location available to all caregivers at all times has been determined and labeled.

Menus. When meals or snacks are provided by the family day home, the menu for the current one-week period shall be given to parents or posted or placed in an area accessible to parents. The menu that was posted was not for the current week.
Plan of Correction: When applicable, ail menus will be current, clearly posted, and visible to parents.
