Inspector Notes

An unannounced renewal inspection was conducted from 11:00 am - 12:00 noon with the provider. There were six children, ranging from three years to five years of age, for a total of 11 points. Children were observed playing with a variety of age-appropriate toys in centers, and eating lunch, which is brought from home. Two children's records and four provider/household member records were reviewed. The emergency drill log, daily attendance, and carbon monoxide detector were observed and in compliance. The provider is not Medication Administration Training (MAT) certified and cannot administer medication to children in care.

Based on a review of two children?s records, the provider did not obtain documentation that each child has received a physical examination prior to the child's first attendance or within one month after attendance. Evidence: the record for child #1 (date of first attendance: 7/5/22) did not have documentation of a physical examination.

Standard 8VAC20-800-280
Based on observation, it was determined that not all potentially poisonous substances were stored in areas inaccessible to children in care. Evidence: there was a canister of Clorox wipes sitting on a low book shelf in the day care area.

Based on observation, and interview, the provider failed to ensure that small electrical appliances were unplugged while not in use. Evidence: there were several small appliances sitting on the kitchen counter plugged in, and the appliances were not in use.
