Inspector Notes
An unannounced, on-site focused monitoring inspection was conducted on 10/30/24, as a part of the provisional licensure period and Intensive Plan of Correction (IPOC). The Department requested that the licensee complete an IPOC as result of recent inspection findings related to forbidden actions and behavioral guidance. The on-site inspection began at 1:35pm and ended at 3:35pm. The inspector reviewed compliance in the areas listed above. The inspector reviewed 3 staff records while on-site. This inspection included document review and interviews. The information gathered during the inspection determined no violations with applicable standards or law. No violations were issued.
