Standard 8VAC20-780-340-A
When staff are supervising children, they shall always ensure their care, protection, and guidance.

On 11/5/25, staff were unaware that they had left Child #1 (a 3 year-old) alone and unsupervised on the 3rd floor outdoor play deck for approximately 6 minutes. The perimeter barrier of the play deck is approximately 6' in height and included 32" of concrete barrier base which holds the chain link fence. At one point, while unsupervised, the child was standing on top of a plastic dome toy and looking over the concrete portion of the barrier.
Plan of Correction: Advisory noticed to TCS staff and Memo to APS staff. Re-training for all staff around sight and sound supervision. Retraining documents in staff files for TCS staff in that particular classroom.

Children under 10 years of age shall always be within actual sight and sound supervision of staff.

On 11/5/25, Child #1 (a 3-year-old), was not in the sight and sound supervision of staff for a period of 6 minutes. Staff #1, #3, #5 and #9 left the child alone and unsupervised on the 3rd floor outdoor play deck.
Plan of Correction: See above; that particular classroom will now complete a "name to face" attendance tracker before and after transitioning from a location to a new location. That classroom will now have a teacher be "the sweeper" before leaving a location.

The operational responsibilities of the licensee shall include ensuring that the center's activities, services, and facilities are maintained in compliance with these standards, the center's own policies and procedures that are required by these standards, and the terms of the current license issued by the department.

On 11/5/25, staff failed to implement the head counting policy to ensure that all children are accounted for before leaving the outdoor play deck or when returning to the classroom.
Plan of Correction: Reviewed the playground policy and headcounts with all staff (TCS and IS). All headcounts for that particular classroom will include name to face checks on tracker with photo.
