On 11/2/25, Child #1 (11-months old), was sitting in a bouncy seat when the child leaned forward in an attempt to get up and flipped forward catching themself with both hands on the floor. The child was buckled into the bouncy seat and the seat base remained upright on the floor with the child's legs underneath of the seat. The manufacturer warning label attached to the bouncy seat stated: "To prevent falls and suffocation: Stop using bouncer when baby starts trying to sit up or has reached 20lb, whichever comes first."

Plan of Correction: All infant equipment was checked in all classrooms. Equipment that was not appropriate for age, skill, or weight was removed.
