Standard 8VAC20-780-160-C
At least every 2 years from the date of the first initial TB screening/testing staff members and individuals from independent contractors shall obtain and submit the results of a follow-up TB screening.

The most recent TB test for Staff #1 was completed 8/10/23.
Plan of Correction: Staff member #1 was given the deadline of January 4th, 2026 (the last day of Winter break) to complete TB screening.

Repeat violation.
Hazardous substances such as cleaning materials, insecticides, and pesticides shall be kept in a locked place using a safe locking method that prevents access by children.

Cleaning supplies were found to be in unlocked cabinets in 2 of 8 classrooms.
Plan of Correction: All staff will be retrained in hazardous substance licensing standards (retraining to occur when staff return from Winter break).

Electrical outlets shall have protective covers that are of a size that cannot be swallowed by children.

Electrical outlets in 3 of 8 classrooms and 1 bathroom used by children did not have protective safety covers.
Plan of Correction: Classrooms were instructed to check all outlet covers and put covers on all outlets, even if up high. Admin completed classroom check December 19, 2025.

Children's hands shall be washed with soap and running water or disposable wipes before and after eating meals or snacks.

Children did not was hands after eating snack in one classroom. Children went from eating snack to a circle time activity.
Plan of Correction: All staff will be retrained in handwashing licensing standards (retraining to occur when staff return from Winter Break).
Standard 8VAC20-780-530-A-1
Current certification in CPR as appropriate to the age of the children in care from an organization such as the American Red Cross, American Heart Association, American Safety and Health Institute, or National Safety Council. The training shall include an in-person competency demonstration.

The CPR course completed by Staff #4 did not include an in-person competency component. It was an online only course.
Plan of Correction: Staff #4 given deadline of January 4th (the last day of winter Break) to complete in-person competency course.
Standard 8VAC20-780-570-A
Repeat violation.
High chairs, infant carrier seats, or feeding tables shall be used for children under 12 months who are not held while being fed and the protective belt shall be fastened securely.

An infant that was being fed at a feeding table was not fastened into the seat. The safety belt was hanging down unfastened.
Plan of Correction: All staff will be retrained in buckling children in when eating (under 12 months of age); retraining to occur when staff return from Winter break.
