Inspector Notes

A monitoring inspection was conducted on site on October 18, 2022 from 9:30 A.M.-11:00 A.M. and concluded on October 19, 2022. There were four children present, ranging in ages from six months to 18 months, for a total of 15 points, with 1 caregiver supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of four child records and one caregiver record were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Based on review of children records, the provider failed to ensure a review with the parent of emergency contact information was completed at least annually.

Evidence:
1. The date documented on Child' # 1's record of the last review of the emergency contact information was 09/07/2021.
2. The provider verified the date of the last review of information.
Plan of Correction: The provider will contact the parents and request that they review the documentation and sign and date the proper paperwork.
