Inspector Notes

An unannounced complaint inspection was conducted on site on 7/25/2022. The inspectors arrived at 12:00 PM and departed at 1:45 P.M. Follow up interviews were completed on 7/27/2022. A complaint was received in the licensing office on 7/20/2022. The preponderance of evidence of sight and sound supervision was supported and complaint is valid.

Based on observation, the center failed to ensure that all areas and equipment was maintained in a safe and operable condition.

Evidence:

The gate of the middle playground that is used by the children was laying on the ground. The cement footer was exposed. This posed a safety hazard to the children in care.

Based on interviews with staff, the center failed to ensure that all children were within sight and sound supervision of a staff member.

On 7/20/2022, a 5 year old child # 1, walked around the elementary school building and through an opening in the fence. The gate was laying on the ground. The child walked around the building to the back porch. One of the elementary school teachers saw child # 1 and took the child into the building and back to class. During this time staff # 3 was returning from being inside and started to search for child # 1. Verified through interviews with all three staff, child # 1 was out of sight and sound supervision for approximately 5 minutes.

Based on interviews and attendance records, the center failed to ensure that the staff to child ratio was met.

Evidence:

On 7/20/2022 there were 40 children ages 2 ? to 5 years of age on the middle playground. There were 2 staff members supervising the children. The required ratio is for one staff for eight children. Five staff members were required.
