Inspector Notes

An unannounced non mandated inspection was conducted on 06/28/2023. There were 81 children, ages 5 months-5 years, and 10 staff members present during the inspection. The inspector observed the following: free choice activities, large motor play outside, and mealtime. A total of 5 staff records, were reviewed.
The inspector arrived at 10:45 AM and departed at 12:15 PM.

Based on interview and review of staff records,
the center failed to ensure that all staff had a
negative tuberculosis (TB) screening at the
time of employment, no more than 30 days
prior to hire, and before coming in contact with
children.

Evidence:

Staff # 4 had a documented employment date of 05/08/2023 and the TB screen was not completed no more than 30 days prior to hire and before coming into contact with children. The TB screening was dated 02/02/2023.
