Inspector Notes

An unannounced monitoring inspection was conducted on 09/03/2024 from 9:25 AM to 10:05 AM. There were three children present, ranging in age from two to three years old with one staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. Three child records were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the deficient practice will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 09/13/2024. A POC submitted after this date will not appear on the public website.

Based on review of records, the provider failed to obtain documentation of additional immunizations every six months for children under the age of two.
Evidence:
1. Documentation of immunizations were last obtained for Child #1 on 05/17/2024 at which time the child was one year old.
2. Documentation of immunizations were obtained for Child #2 on 01/25/2022 at seven months old and 06/18/2023 at three years old.
3. Documentation of immunizations were obtained for Child #3 on 08/04/2022 at four months old, 2/21/2023 at ten months old.
