Inspector Notes

An unannounced, on- site monitoring inspection was initiated on September 5, 2025 and completed on September 5, 2025. The on-site inspection began at 10:40 a.m. and ended at 11:50 a.m. The inspector reviewed compliance in the areas listed above. There were four children present and one caregiver; the point total was six. The inspector reviewed five children?s records and one caregiver/household member records on-site . This inspection included document review, tour of the facility, interviews and

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Providers shall have current certification in cardiopulmonary (CPR) from the American Red Cross, American Heart Association, American Safety and Health Institute, or the National Safety Council or current CPR certification issued within the past two years by a community college, a hospital, a rescue squad, or a fire department.

A child?s record shall contain all of the required information.
The record for Child #1, enrolled for two weeks, was missing the name, address and telephone number for each custodial parent?s place of employment; the address of one of the designated persons to be contacted in case of emergency; and the first date of attendance.
The record for Child #2, enrolled for four months, was missing name, address and telephone number for one parent; name, address and telephone number of one parent?s place of employment; and the phone number of the second parent?s place of employment.
The record for Child #3, enrolled for three weeks, was missing the name, address and telephone number of the place of employment for both parents; and the first date of attendance.
