Inspector Notes
An unannounced, on- site monitoring inspection was initiated on September 5, 2025 and completed on September 5, 2025. The on-site inspection began at 10:40 a.m. and ended at 11:50 a.m. The inspector reviewed compliance in the areas listed above. There were four children present and one caregiver; the point total was six. The inspector reviewed five children?s records and one caregiver/household member records on-site . Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on September 16, 2025. A POC submitted after this date will not appear on the public website.

The provider?s current first aid certification was not from one of the approved organizations.
Cleaning supplies were being stored in an unlocked cabinet under the kitchen sink.
One child was observed using the trampoline during the inspection.
A child?s record shall contain all of the required information.
The record for Child #1, enrolled for two weeks, was missing the name, address and telephone number for each custodial parent?s place of employment; the address of one of the designated persons to be contacted in case of emergency; and the first date of attendance.
The record for Child #2, enrolled for four months, was missing name, address and telephone number for one parent; name, address and telephone number of one parent?s place of employment; and the phone number of the second parent?s place of employment.
The record for Child #3, enrolled for three weeks, was missing the name, address and telephone number of the place of employment for both parents; and the first date of attendance.
The record for Child #4, enrolled for three weeks, was missing the telephone number of the place of employment for both parents; and the address of one of the designated persons to be contacted in case of emergency.
The record for Child #5, four months, was missing the telephone number of the place of employment for one parent; and the address of both designated persons to be contacted in case of emergency.
Several medications were being stored in an unlocked kitchen cabinet.
