During the inspection, the inspector reviewed the areas listed above. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced inspection was initiated on October 23, 2025 and completed on November 6, 2025 in response to a complaint received by the licensing office on October 7, 2025 relating to allegations of Care of Children and Transportation. The inspector reviewed three children?s records on-site. This inspection included document review and interviews.

The preponderance of evidence gathered during the investigation supports the allegations; therefore, the complaint is determined to be valid. Information gathered during the inspection determined non-compliance with applicable standards or law and the violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on November 19, 2025. A POC submitted after this date will not appear on the public website.

Standard 8VAC20-800-1010
The provider shall ensure that during transportation of children, that no child is left unattended inside or outside a vehicle.
On October 7, 2025, the provider left three children (ages two to three years old) unattended in a vehicle while the provider went inside of a store.

Standard 8VAC20-800-990-B
The provider shall ensure that during transportation of children the driver has a copy of the parent?s written permission to transport the child.
On October 7, 2025, the provider transported two children to the Dollar General and three children from the Dollar General back to the home. The provider did not have written permission to transport the children to or from the Dollar General.
