Inspector Notes
This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review, interviews tour of the program.
A complaint inspection was initiated on 7/28/2021 and concluded on 9/3/2021. There were 7children present, ranging in ages from five months to seven years, with two staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, special care and emergencies and nutrition. A total of two child records and one staff record were reviewed.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.
Violations
4Evidence:
1. There is a six foot blanket ladder in the nap room, if tipped over it could injure a child.
2. There is an industrial size extension cord plugged into the outlet in the nap room and is threaded through the window and hooked up to a camper outside.
3. The paint is peeling on the walls in the nap room and the door to the nap room is splintered.
Evidence:
1. The bathroom in the nap room has all the cleaning agents stored on the floor, the door is open and the substances are accessible to the children.
Evidence:
1. The provider was requiring the children to put used toilet paper in the trash can.
Evidence:
1. The provider stated that the assistant cares for the children when she has to run errands or has a doctor appointment. There is no documentation in the children's records stating the name of the assistant or the amount of time she is caring for the children.
