Inspector Notes

This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review.

A monitoring inspection was initiated on 9/24/2021 and concluded on 10/12/2021. The provider was contacted by telephone and a virtual inspection was conducted. There were 6 children present, ranging in ages from eight months to five years, with two staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of three child records and two staff records were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Based on record reviews, the provider failed to obtain documentaion of a physical examination withi 30 days after the first day of attendance.
Evidence:
1. The inspector reviewed three children's records. Child Record #2 did not contain documentation of a physical exam and child's first day of attendance was 12/14.2021.
Plan of Correction: The provider will ensure that children's physicals are received within 30 days of the first day of attendance.
