Inspector Notes

A monitoring inspection was initiated on 9/18/2023 and concluded on 9/18/2023. There was one child present with one provider. The point total was two. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of two child records and one staff records were reviewed. The inspection started at 12:20 pm and concluded at 1:50 pm.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Violations

Standard 22.1-289.011-F
Based on observation the provider failed to ensure that the license and any other documents required by the Superintendent shall be posted in a conspicuous place on the licensed premises.
Evidence:
The violation notice that was posted was dated 10/3/2022.
Plan of Correction: The provider will ensure that the most current violation notice is posted.
Standard 22.1-289.036-A
Based on record reviews the provider failed to ensure that every applicant for licensure as a family day home, adult living in such family day home shall undergo a background check in accordance with subsection B prior to issuance of a license , and every five years thereafter.
Evidence:
Staff #1 and HHM #1 have a fingerprint background check that expired 5/16/23 and 5/15/23.
Plan of Correction: The provider will schedule appointments with OBI to have the fieldprint background checks completed. The provider will ensure that all repeat background checks are completed prior to the five year expiration dates.
Standard 8VAC20-800-140-B-3
Based on record reviews the provider failed to ensure that providers licensed after June 30, 2010, shall have: Current certification in cardiopulmonary resuscitation (CPR), as appropriate to the age of the children in care, from the American Red Cross, American Heart Association, American Safety and Health Institute, or the National Safety Council, or current CPR certification issued within the past two years by a community college, a hospital, a rescue squad, or a fire department.
Evidence:
Staff #1's CPR certification expired 3/20/23.
Plan of Correction: The provider will register for First Aid and CPR training.

Based on record reviews the provider failed to ensure that current certification in first aid from the American Red Cross, American Heart Association, American Safety and Health Institute, or the National Safety Council, or current first aid certification issued within the past three years by a community college, a hospital, a rescue squad, or a fire department.
Evidence:
Staff #1 has a first aid certification that expired 3/20/23.
Plan of Correction: The provider will register for First Aid and CPR training.
Standard 8VAC20-800-280
REPEAT VIOLATION
Based on observation the provider failed to ensure that potentially poisonous substances, materials and supplies such as, but not limited to, cleaning agents, disinfectants, deodorizers, plant care chemicals, pesticides, and petroleum distillates shall be stored away from food in areas inaccessible to children.
Evidence:
Located in the nap room is a bathroom that is being used as a storage space for cleaning supplies. The door is not locked and there was also cleaning supplies, disinfectants on the floor of the nap room. A child was on a cot in this room and had access to all of the cleaning supplies that were not locked.
Plan of Correction: The provider is reorganizing the two rooms used for child care. The provider will ensure that the children will not be in rooms with hazardous substances.
Standard 8VAC20-800-460-D
Based on record reviews the provider failed to maintain documentation of the current rabies vaccination.
Evidence:
There is a dog at the family day home that is there during the hours that children are in care. The rabies vaccination provided for review expired 8/2023.
Plan of Correction: The provider will ensure that any pets that are around the children will have up-to-dates rabies shots.
