Inspector Notes

An unannounced monitoring inspection was conducted on 8/26/22 from 10:30am - 11:30am. During the inspection there were 48 children ages one year old through five years old in care with 15 staff. Children were observed participating in various activities in the classroom. Records were reviewed for 20 staff during the inspection. There is no medication at the facility. Emergency procedures and emergency supplies were reviewed during the inspection. Information gathered during the inspection determined non-compliance(s) with applicable standards or law and violations were documented on the violation notice issued to the program, and were discussed during the exit interview.

Based on a review of 20 staff records, it was determined that the facility did not deny employment of a staff who did not have a criminal record check from any other state in which the employee has resided in the preceding five years.

Evidence:
1. The record for staff #18 did not contain documentation of a completed criminal record check from Alabama. The sworn statement indicated staff #18 had previously lived in Alabama within the last 5 years.
2. Staff #19 reviewed the record for the staff #18 and confirmed that the criminal record check from the state (Alabama) in which staff #18 previously resided in has not been received prior to employment.
Plan of Correction: The facility responded: We thought we had sent off the correct background checks for staff #18, but discovered we forgot one. The missing background check has been sent off for staff #18 and will be forwarded to the Licensing Inspector when completed.
Standard 8VAC20-770-40-D-2
Based on a review of 20 staff records, it was determined that the facility did not deny continued employment of a staff who did not have a search of the central registry finding within 30 days of employment.

Evidence:
1. The record for staff #15 did not contain documentation of a completed search of the central registry finding.
2. Staff #19 reviewed the record for the staff #15, and confirmed that the search of the central registry finding had not been received within 30 days of employment.
Plan of Correction: The facility responded: The search of the central registry was sent off for staff #15 previously. It was returned for an error but we did not receive it. The corrections have been made and it has been resent and will be forwarded to the Licensing Inspector when completed.
