Inspector Notes

An unannounced monitoring health and safety inspection was conducted on June 16, 2022 to review supplemental health and safety requirements. The inspector arrived at 11:25 AM and concluded the inspection at 12:20 PM. Today, there were eleven children, totaling nineteen points, present with 2 caregivers. During the inspection, the children were observed playing with appropriate materials, and sitting in high chairs. Children?s equipment, learning materials, indoor space, kitchen, and restrooms were inspected. Five children's records, one provider record, one assistant record, daily attendance records, fire drill and shelter in place drill logs, and all required postings were reviewed. The provider was available for the inspection and was present at the exit interview at which time inspection findings were reviewed and an Acknowledgement of Inspection form was signed and left with the licensee.

A licensing inspection was also conducted this date; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection. Violations were found during the supplemental subsidy health and safety inspection and are identified on the violation notice.

If you have any questions about this inspection, please contact the licensing inspector, call (804) 588-2362 or email, LaTasha.Smith@doe.virginia.gov

Evidence: 1. Caregiver #2 did not have documentation of completing the Virginia Preservice Training within 90 days from approval as a subsidy vender dated 02/21/2019. 2. The provider stated caregiver #2 had not completed the Virginia preservice training.
Plan of Correction: We will start the training this weekend.
