Inspector Notes
A renewal inspection was conducted on November 15, 2022 from 9:40 am until 11:30 am. There were sixteen children present, ranging in ages from two to five years old, with three staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of three child records and three staff records were reviewed.
Information gathered during the inspection determined noncompliance with applicable standards or law and violations were documented on the violation notice issued to the facility.
Please complete the plan of correction and date to be corrected sections for each violation cited on the violation notice and return it to me within 5 business days from today. Please specify how the deficient practice will be or has been corrected. Your plan of correction should contain: 1) steps to correct the noncompliance with the standard(s): 2) measures to prevent the noncompliance from occurring again; and 3) person(s) responsible for implementing each step and/or monitoring any preventive measure(s).

Violations
2Evidence:
1. The first day of employment for Staff #3 was 11/1/2022. There were no criminal history results in the file for a previous state of residence.
2. The Director confirmed that she had received criminal history results for Virginia prior to the start of employment, but had not requested results from the previous state of residence.
Evidence:
1. During snack time on 11/15/2022, an unlabeled, reusable thermos was identified. Staff questioned one another to see if anyone knew who the owner was. When they were not certain of the owner, it was placed on top of the cubbies and was not given to any child.
2. The Director confirmed that it was not labeled and that they tell parents it is required, but that it was a new thermos.
