Inspector Notes

This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review, interviews and a virtual tour of the program.

A monitoring inspection was initiated on 12/28/2021 and concluded on 01/12/2022. The director was contacted by telephone and a virtual inspection was conducted. There were 8 children present with two staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, special care and emergencies and nutrition. A total of two child records and two staff records were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Standard 22.1-289.035-A
Based on a review of two staff records and interview, the center did not obtain a repeat background check for one staff member every five years.

Evidence: 1) The record for Staff #2, hired on 03/01/2016, contains documentation of a central registry finding dated 02/11/2016. The central registry finding should have been updated by 02/11/2021. 2) During interview, a member of management verified the repeat central registry finding for Staff #2 was not received by 02/11/21.
Plan of Correction: The request for Staff #2 was resubmitted on 01/10/22. In the future, repeat background checks will be conducted within the required time frame.
Standard 8VAC20-780-130-A
Based on a review of two children's records and interviews, the center did not obtain documentation that one child had received the immunizations required by the State Board of Health before the child attended the center.

Evidence: 1) The immunization documentation in the record for Child #2, enrolled on 11/15/21, was dated 11/19/21. 2) During interview, a member of management confirmed the center did not have any other immunization documentation on file for Child #2.
Plan of Correction: In the future, all immunization documentation will be maintained by the center for the child's record. Immunizations will be obtained prior to a child's first day of attendance.
