Inspector Notes
An unannounced monitoring inspection was conducted on Thursday, February 22, 2024 to determine the provider's compliance with licensing standards. Two inspectors were on site from 10:20am to approximately 1:15pm. There were 25 children in the direct care of eight staff. Upon the inspectors' arrival, the children were observed in their designated classrooms. They were observed during free-play, independent play, and teacher-led activities. Later, the preschool children were observed playing outside. The center is equipped with age-appropriate materials and equipment for the children's use. Staff were engaged with the children and offering guidance when needed. The areas where children receive care were inspected and found to be in compliance. The center had the following posted: license, daily schedule, emergency numbers, evacuation maps, and various parent information. The center's first aid kit and non-emergency supplies were inspected and found complete. During the inspection, five children's records and five staff records were reviewed. The center administers medications when needed and medications and authorizations were reviewed. Current background checks for applicants were reviewed.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.
Violations
7Evidence: The most recent national fingerprint-based criminal background check in the record of Applicant #1 was obtained on 09/08/23. The previous national fingerprint-based criminal background check was obtained on 08/14/18 and should have been repeated no later than 08/14/23.
Evidence: The physical examination in the record of Child #4 (DOA: 08/14/23) was not signed by a physician, his designee, or an official of a local health department.
During interview, a member of management reported the center did not have any other documentation on file for Child #4.
Evidence: The TB screening in the record of Staff #1, employed on 01/23/24, was completed on 10/24/23.
During interview, a member of management confirmed the TB screening was not completed within the required timeframe.
The documentation shall have been completed within the last 30 calendar days of the date of employment and be signed by a physician, physician's designee, or an official of the local health department.
Evidence: The record of Staff #5, employed on 09/01/23, did not contain the Virginia Department of Education-sponsored orientation course.
During interview, a member of management reported the staff member has not completed the orientation to date.
The Virginia Department of Education-sponsored orientation course shall be completed within 90 calendar days of employment.
Evidence: The record of Staff #2, employed on 01/23/24, and the record of Staff #4, employed on 02/12/24, did not contain documentation that the staff had completed orientation training.
During interview, a member of management acknowledged the orientation training for Staff #2 and Staff #4 was not completed within the required timeframe. The training would be completed for both staff members on 02/22/24.
Evidence: The following children's records did not contain documentation of child updates and confirmation of up-to-date information in the child's record as required by 8VAC20-780-420 E 3: Child #2 (DOA: 01/06/20), Child #3 (DOA: 01/02/23), and Child #5 (DOA: 07/03/17).
During interview, a member of management acknowledged the records did not contain confirmation of up-to-date information.
Evidence: A reference check in the record of Staff #4, employed on 02/12/24, was obtained on 02/13/24.
During interview, a member of management acknowledged the reference check was obtained after Staff #4 started employment.
Each staff record shall contain documentation that two or more references as to character and reputation as well as competency were checked before employment or volunteering.
