Inspector Notes

This inspection was conducted on site by licensing staff,.

A renewal inspection was initiated on March 29, 2022 and concluded on March 29, 2022. The director was in attendance and participated with the inspection. There were 16 children present, ranging in ages from nine months to four years, with six staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of three child records and four staff records were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Based on observation and interview, the center did not ensure the center was equipped with at least one carbon monoxide detector.

Evidence:

The center did not have a carbon monoxide detector. The administration stated they had not installed a carbon monoxide detector.
Plan of Correction: Per the administration: We will purchase and install a carbon monoxide detector.
Standard 8VAC20-780-140-A
Based on review of three children's records, the center did not obtain documentation of a physical examination before the child's attendance or within one month after attendance for one child's record.

Evidence:

The record of child #3, enrolled 11/22/2021, did not contain documentation of a physical examination exceeding the required time period.
Plan of Correction: Per the administration: We will ask the parent to provide documentation of the child's physical for the record.
Standard 8VAC20-780-420-E-1
Based on review of the center's brightwheel application and interview, the center did not maintain a daily record for each infant that contained the amount of time the infant slept; the amount of food consumed and the time; a description and time of bowl movements; and developmental milestones.

Evidence:

The brightwheel application did not have the required daily information for infant #4 and infant #5 documented the week of March 21 through March 28, 2022. Staff #3 stated she had not recorded the required information.
Plan of Correction: Per the administration: We will document in brightwheel all the required information as it occurs for each infant in care.
