Inspector Notes

This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review and interviews.

A renewal inspection was initiated and concluded on 11/22/2021. The provider was contacted by telephone and a virtual inspection was conducted. There were 5 children present, ranging in ages from 15 months to 4 years of age with 1 staff supervising. The inspector reviewed compliance in the areas of administration, staffing and supervision, programming, special care and emergencies and nutrition. A total of 4 child records, 1 staff record and 3 household member records were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Based on record review and interview, children's records were not reviewed annually.
Evidence: Child #1 (date of enrollment: 1/8/2019) record did not contain documentation that the child's record had been reviewed annually to ensure all contact information was up-to-date.
Plan of Correction: I will have the parents review this record when they return from vacation. I will start reviewing records on a regular basis, maybe twice a year, to make sure all information is accurate.

Based on record review, the provider has not obtained verification of the age and identity of each child within the first seven business days of enrollment.
Evidence:
1. Child #2 (date of enrollment: 6/1/2021) file does not contain documentation that the child's identity has been verified.
2. Child #4 (date of enrollment: 11/2/2020) file does not contain documentation that the child's identity has been verified.
Plan of Correction: I will have the parents bring the birth certificates this week. I will start requiring families to provide this before enrollment to make sure the records are complete before the child starts.
