Inspector Notes
An unannounced renewal inspection was conducted today between 11:40am and 1:15pm. There were 5 children present with the Provider and 1 assistant for a total of 16 points within the supervision guidelines. The children were observed playing freely, washing hands, eating lunch. A complete inspection of the physical plant, children and staff records, fire drill log, menu, attendance and emergency procedures were observed during this inspection. Violations were found during today?s inspection and can be viewed on the Violation Notice.

Violations
11Evidence: Caregiver #2s central registry background check was dated 4/12/2017 and had expired on 4/12/2021. The expired central registry background check was not renewed until 11/18/2022.
Evidence:
1. Caregiver #1's TB screening expired on 9/10/2022 and was renewed on 9/16/2022.
2. Caregiver #2's TB screening expired on 6/15/2021 and was renewed on 10/24/2022.
3. Household member #1's TB screening expired on 12/5/2022 and has not been renewed.
Evidence:
1. Caregiver #2 has not completed any training since 2019.
2. Caregiver #3 and Caregiver #4 have not completed any training.
Evidence: At the start of the inspection, the provider had not yet completed attendance for the day. When asked when the most recent inspection was completed, the provider showed an attendance sheet dated from the week prior. The last day the provider had documented attendance was 12/9/2022.
Evidence: Child #1 (date of enrollment: 6/1/2021) and Child #5 (date of enrollment: 7/5/2021) records have not been reviewed for accuracy since enrollment.
Evidence:
1. Child #2's record does not contain documentation that the family had been informed of the providers medication policy, emergency preparedness plan, or insurance declaration.
2. Child #3's record does not contain the families medical insurance policy number.
3. Child #4's record does not contain the child's first date of attendance or the families medical insurance policy number.
4. Child #5's record does not contain the families medical insurance policy number.
Evidence: The most recent review of the emergency preparedness plan was dated 8/5/2020.
Evidence: There is no documentation of an evacuation drill practiced in January 2022. The provider confirmed that a drilled had been forgotten.
Evidence: Child #3 (18 months) most recent immunizations are dated from when the child was 6 months.
Evidence: The weekly menu was not dated.
Evidence: The inspection posted at the family day home is dated 12/7/2020, however the most recent inspection was on 11/21/2021.
