Inspector Notes

An unannounced renewal inspection was conducted today between 11:40am and 1:15pm. There were 5 children present with the Provider and 1 assistant for a total of 16 points within the supervision guidelines. The children were observed playing freely, washing hands, eating lunch. A complete inspection of the physical plant, children and staff records, fire drill log, menu, attendance and emergency procedures were observed during this inspection. Violations were found during today?s inspection and can be viewed on the Violation Notice.

Based on record review and interview, the provider did not obtain a background check for each employee every five years.
Evidence: Caregiver #2s central registry background check was dated 4/12/2017 and had expired on 4/12/2021. The expired central registry background check was not renewed until 11/18/2022.

Based on record review, each caregiver and/or adult household member has not obtained a repeat tuberculosis (TB) screening every two years.
Evidence:
1. Caregiver #1's TB screening expired on 9/10/2022 and was renewed on 9/16/2022.
2. Caregiver #2's TB screening expired on 6/15/2021 and was renewed on 10/24/2022.
3. Household member #1's TB screening expired on 12/5/2022 and has not been renewed.
