Inspector Notes

An unannounced, on-site monitoring inspection was initiated on 10/21/2024 and completed on 10/21/2024. The on-site inspection began at 12:40PM and ended at 1:45PM. The inspector reviewed compliance in the areas listed above. There were 10 children present and 3 caregivers; the point total was22. The inspector reviewed 3 children?s records and 5 caregiver/household member records. This inspection included document review, a tour of the family day home, interviews and observations. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program. Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the deficient practice will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 10/31/2024. A POC submitted after this date will not appear on the public website. Standard 22.1-289.035-B-2
Providers must obtain a completed national criminal background check prior to the employee's first day of employment.

The background check for Caregiver 1 was completed 1 month after their first date of employment.
Plan of Correction: Will make sure future employees' background checks will be completed before employment.
Standard 22.1-289.036-A
The Family Day Home is required to have household members to undergo background checks every 5 years.

Household member 1 and household member 2 each had an expired background check by over one month. Household member 3 had an expired background check by over 10 months.
Plan of Correction: Household member 1 & 2 have submitted the central registry checks. Household member 3's was misplaced and is now in the file as of 10/24/2024.

For staff hired before July 1, 2024, providers must obtain the results of a central registry search for an employee, within 30 days of service.

Caregiver 1 did not have central registry search results during the inspection and has been employed for over 45 days.
Plan of Correction: Central Registry search for caregiver 1 has been submitted. Awaiting search results.
Standard 8VAC20-800-170-A
Providers must obtain a current tuberculosis screening from a caregiver at the time of hire and prior to coming into contact with children.

There was no tuberculosis screening for Caregiver 1, who has been caring for children for over one month.
Plan of Correction: Screening reports have been completed and emailed to the inspector.
Standard 8VAC20-800-180-A
A tuberculosis (TB) screening is required every two years from the last screening.

An updated TB screening for household member 1 & household member 2 was not completed and was overdue by three months.
