Inspector Notes

This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review, interviews and a virtual tour of the program .

A renewal inspection was initiated on 02/17/2022 and concluded on 03/02/2022. The director/provider or in-charge person was contacted by telephone to initiate the inspection. There 112 children and 16 staff present. The inspector emailed the director a list of items required to complete the inspection. The Inspector reviewed 5 children?s records and 5 staff records, along with any requested program records submitted by the facility to determine if required documentation was complete.

Information gathered during the inspection determined non-compliance(s) with applicable standards or law and violations were documented on the violation notice issued to the program.

Based on record review and interview, it was determined that the center did not ensure that employee that had lived out of the state of VA within the last 5 years had a results of an criminal history record information check, out-of-state Central Registry results and sex offender results.

Evidence:
Staff #1 hired date was 10/20/2021 lived out of the state of VA within the past 5 years did not have results for Out of State Central Registry for California.

Staff #2 hire date 08/31/21 lived out of the state of VA within the past 5 years did not have results of a out of state central registry results for Florida.

Staff #4 hire date 9/21/2021 lived out of the state of VA within the past 5 years did not have results of a out of state central registry results for New York.
