Inspector Notes

An unannounced monitoring inspection was conducted on Septemeber 08, 2022 from approximately 10:30 am-12:45pm. There were 111children present at this time with 18 staff members. Additional staff were on site for administrative and support services. Children were observed participating in station activities, playing with puzzles, coloring, outside play and potty training/diapering . Nine staff records, seven children's records, 6 injury reports and 5 medications were reviewed during the inspection. Licensing Inspector reviewed indoor areas, supervision, activities, equipment, medication procedures, emergency evacuation and shelter-in-place drill documentation, emergency procedures and required posting.

Information gathered during the inspection determined non-compliance(s) with applicable standards or law and violations were documented on the violation notice issued to the program.

Based on record review 1 of 9 staff records, the licensee did not ensure that employee that had lived out of the state of VA within the last 5 years had a results of an criminal history record information check before employed.

Evidence:
Staff #1 hired date was 4 /11/2021 lived out of the state of VA within the past 5 years. The out-state criminal history record check was submitted to the state of Mass. , there was documentation of follow up of emails and phone calls for the Criminal history record check. The center has not recieved results. The center confirmed staff #1 is working at the facility. Staff #10 was continuing to contact the state of Massachusetts.

Based on 4 of 9 records reviewed, the licensee did not ensure staff documentation of tuburculosis (TB) screening shall be submitted at the time of employment and prior to coming in contact with children; and the T.B.documentation have been completed with in the last 30days of the date of employment.

Evidence:

Staff #1 hire date 08/15/2022, most recent TB test 12/06/2021. The TB test was more than 30days prior to employment.

Staff #2 hire date 08/08/2022 , most recent TB test 12/04/2020. The TB test was more than 30days prior to employment.

Staff #4 hire date 08/02/2022, most recent TB test 02/19/2022.The TB test was more than 30days prior to employment.

Staff #6 hire date 08/31/2022, the TB documentation was not available during the inspection.
